You should only make a declaration if you personally are paying income tax and/or capital gains tax at least equal to the tax the Scottish Council on Human Bioethics will reclaim on your donations in the tax year (currently 28%).
If your name, address or tax situation changes, please notify the Scottish Council on Human Bioethics immediately.
If you pay tax at the higher rate you can claim further tax relief in your Self-Assessment tax return.
